if i mentioned cisz(4096), then this 4k memory is for data cluster or index cluster or for both?

It is usually the DATA Clusters CI that we will be mentioning in the IDCAMS statement. If you are in doubt you can always check it once it is created using the LISTCAT IDCAMS COMMAND.
