Can anyone suggest me how to use the UNIQUE command on multiple columns ?

Hello,
Please post an example of your "input" data values and what you want "unique" to accomplish as "output". |

I need only on row to be displayed for a set of values for all the feilds.

Sorry, but i do not understand what this tells me. If you want "only one row" what would it contain?
Is there a reason the .jpg is posted twice?
